— having reviewed the application from the administrative division for the assessment of football clubs in the Center for Football Education and Leadership regarding a possible violation of the accreditation system requirements by FC «Poltava» (Poltava), ordered FC «Poltava» (Poltava) to pay an obligatory financial contribution of 400,000 (four hundred thousand) hryvnias (for providing inaccurate information during the accreditation process);

— having reviewed materials regarding possible violations of the Code of Ethics and Fair Play by the Ukrainian Association of Football (UAF) by Andriy Zaporozhan before the match between FC «Ruh» (Lviv) — FC «Left Bank» (Kyiv), which took place on November 24, 2024, prohibited Andriy Zaporozhan from engaging in any football-related activity (administrative, sports, etc.) for a period of three years (for failing to comply with the requirements of the UAF Code of Ethics and Fair Play, namely: not offering, promising, giving or accepting directly or indirectly any unwarranted financial assistance or other benefits in any form with the aim of influencing actions or inactions within their competence; refraining from any behavior that harms or may harm the integrity of matches and competitions; not using methods and means in their activities that could have an illegal or improper influence on the course and/or result of a match or competition for personal gain or the benefit of a third party);

— having reviewed materials regarding possible violations of the UAF Code of Ethics and Fair Play by Dmytro Makhnev, prohibited Dmytro Makhnev from engaging in any football-related activity (administrative, sports, etc.) for one year (for failing to comply with the requirements of the UAF Code of Ethics and Fair Play, namely: persons participating in competitions or their organization are forbidden to participate directly, indirectly, or otherwise in activities related to betting, totalizator, gambling games, lotteries, similar events or operations that have a connection to sports), conditionally for six months with a probationary period of one year;

— having reviewed materials regarding the behavior of player «Sky Up Futsal» (Kyiv) Andrade de Almeida Joao Lucas after the third match of the final series for bronze medals in the Ukrainian Championship in futsal among teams of the FAVBET Extra League season 2024/2025 «Sky Up Futsal» (Kyiv) — «Sokil» (Khmelnytskyi), which took place on June 14, 2025, disqualified player «Sky Up Futsal» (Kyiv) Andrade de Almeida Joao Lucas from participating in matches for three games (for violent behavior);

— having reviewed materials regarding the behavior of players after the third match of the final series for bronze medals in the Ukrainian Championship in futsal among teams of the FAVBET Extra League season 2024/2025 «Sky Up Futsal» (Kyiv) — «Sokil» (Khmelnytskyi), which took place on June 14, 2025, ordered «Sky Up Futsal» (Kyiv) to pay an obligatory financial contribution of 25,000 (twenty-five thousand) hryvnias (for repeated unethical and unsportsmanlike behavior by players of «Sky Up Futsal» (Kyiv) after the match);

— having reviewed materials regarding the final series playoff match for gold medals in the Ukrainian Championship in futsal among teams of the FAVBET Extra League season 2024/2025 «HIT» (Kyiv) — «Aurora TIM» (Kyiv), which took place on June 14, 2025, ordered «HIT» (Kyiv) to pay an obligatory financial contribution of 5,000 (five thousand) hryvnias (for the absence of the head coach or another official representative of the team at a flash interview), ordered «Aurora TIM» (Kyiv) to pay an obligatory financial contribution of 5,000 (five thousand) hryvnias (for the absence of the head coach or another official representative of the team at a flash interview), disqualified player «HIT» (Kyiv) Serhiy Zhurbu from participating in matches for three games (for using indecent, insulting language towards the match referee), ordered «HIT» (Kyiv) to pay an obligatory financial contribution of 7,500 (seven thousand five hundred) hryvnias (for unethical and unsportsmanlike behavior by a player of «HIT» (Kyiv) during the match) and an obligatory financial contribution of 10,000 (ten thousand) hryvnias (for unauthorized appearance in the playing area by third parties);

— having reviewed materials regarding the final series playoff match for gold medals in the Ukrainian Championship in futsal among teams of the FAVBET Extra League season 2024/2025 «HIT» (Kyiv) — «Aurora TIM» (Kyiv), which took place on June 15, 2025, ordered «HIT» (Kyiv) to pay an obligatory financial contribution of 20,000 (twenty thousand) hryvnias (for repeated unauthorized appearance in the playing area by third parties).
